Reporting to the Team Leader you will be responsible for providing case management services to vulnerable children, young people and their families.
Ensuring assessments, care plans, reviews and exit plans are in line with program guidelines
Develop initial case plans in partnership with the family
Complete assessments and referrals for case management services
Comply with the programs administration and reporting requirements
Provide a high quality of service that is evidence and outcome based
Skills and Experience:
Tertiary qualifications in Social Work or other appropriate qualification
Previous experience in case management in Integrated Family Services
Working with DHHS Best Interest Case Practice and Stability Planning models
Experience working with children and their families from CALD communities
Knowledge of the Children, Youth and Families Act, 2005
